# major issues with completing the square question

For question 23, completing the square, I don't understand where i went wrong. Was I suppose to get half of 5 and square it ?(Punch)

You don't need to complete the square here. It's already done for you.
$$\displaystyle (t + 5)^2 = 12$$

Now take the square root of both sides.
