Start out with this: $\displaystyle y=a(x-h)^2+k$, where [tex](h, k) is the vertex.

$\displaystyle y=a(x+3)^2+3$

Now, substitute your point (-1, 4) in for x and y to solve for a.

Then put it all back together again in the general form $\displaystyle y=ax^2+bx+c$ or vertex form $\displaystyle y=a(x-h)^2+k$.
