Hint: You need to square both terms before adding them together (and finishing off by taking the square root). (x/2)^2 = x^2/4 for example.

(x/2)^2 + (x-1/4)^2

= x^2/4 + (x^2 - 2x + 1)/16
= 4x^2/16 + (x^2 - 2x + 1)/16
= (5x^2 - 2x + 1)/16

Taking the square root of that gives us

SQRT(5x^2 - 2x + 1)/4
