I understand the concept of the uniqueness and existence theorem and have a formulation of it completed for another equation as posted in another thread.

However, I am having difficulty translating that to this problem:

Consider the differential equation x' = f(t, x), with single dependent variable x.
Give the formulation of the existence and uniqueness theorem for the associated initial value problem.

Any guidance would be appreciated.
