b)write a formula for the fraction (ratio) consisting the first number divided by the second number.
c) write an equation that says that this fraction (ratio) is 5/4.

Well, if the smaller number is x and the other number is x+7, I can set up the following ratio

$\overbrace{\frac{x+7}{x}}^\text{first number over second number}=\frac{5}{4}$

Cross multiply, move x terms to one side of the eqution and constants to the other, combine like terms, and then divide each side by the coefficient of the variable.
