$\displaystyle y = \sqrt{t} + 4$

swap variables ...

$\displaystyle t = \sqrt{y} + 4$

solve for y ...

$\displaystyle t - 4 = \sqrt{y}$

$\displaystyle (t-4)^2 = y$

have you learned about restricted domains for an inverse?
