$y = a(x - h)^2 + k$ , where $(h,k)$ are the vertex coordinates.

$y = -\frac{1}{2}(x - 0)^2 + 0$

moving the vertex of the parabola from $(0,0)$ to $(2,-4)$ ...

$y = -\frac{1}{2}(x-2)^2 - 4$
