Subtract x
Square both sides (don't forget to "FOIL"/distribute)
Subtract 5
Check for extraneous roots

If $\sqrt{\sqrt{x+5}+x~}=5$ then $\sqrt{x+5}=25-x$ .
Now you finish it off.
