How to Use this Formula? I have 3 numbers to input. Any ideas?

Im hoping someone can help do this for me. My data is on left of image, the instructions/example are on the right of image under orange text: http://s29.postimg.org/m0fwf679h/Variables.jpg

Construct a 95% confidence interval (Using the following formula: 95% C.I. Equals "mean +/- t*Standard Error" . It also asks for the upper tail and T=, idk how to find that with the t table thing.

You will be using a t-test since you are using data from a sample and you do not know the population standard deviation. You need to calculate the degrees of freedom (df), which equals sample size minus one (df=n-1).

So basically I need these below answered (I already know the mean and standard error I believe, as shown in the image):

    ----------------
    Mean:
    Standard Error:
    t:
    95% C.I.:
    Upper limit:
    Lower limit:

"this case we are ???....... "
