    Linear Algebra Linear transformations isomorphic

Can someone please help me i'm really stuck on this problem.

Let V be a vector space of real-valued polynomials of degree at most 2, and consider the map T:V->R^3, p->[p(0),p(1),p(2)] (should be 3x1 matrix form) , Prove that T is an isomorphism.
