Cartesian form: The cross product of the given vectors will give you a vector that's normal to the plane. Then ax + by + cz = d where a, b and c are the coponents of the normal. d is found by substituting the given point.

If you need more help, please show all your work and say where you're stuck. Personally, I suggest you first review your class notes and textbook for similar examples.
